Feely, Ltd. participates in the BlueCross and Blue Shield PPO Plan. If you have Blue Cross and Blue Shield
PPO coverage, you will be responsible for the deductible and co-paymnent under your plan at the time services
are rendered.
Medicare
We have "opted out" out of Medicare by choice and do not participate with United Health Care plan.
Out-of-Network
At the completion of each visit, we will provide you with a document to assist you in completing your insurance claim form. Please utilize the HCFA 1500 form when filing your own insurance for filing out-of-network benefits. In the event you need an insurance claim form, please contact your benefit administrator at work, your insurance company representative or medical plan carrier.
Worker's Compensation / Disability Evaluation
Any patient being seen for a workers compensation injury and/or disability evaluation must obtain authorization in writing, from your employer or attorney accepting financial responsibility for medical services and treatment. If you have any questions, please speak to our office manager prior to scheduling your appointment with the doctor.
